Prescriptions

To request refills on your regularly prescribed medications, please contact your local or mail-order pharmacy first. In most cases, the pharmacy will send an electronic request directly to your healthcare provider's computer. This process is very efficient and generally allows for a faster response.

Please plan ahead and make the call regarding medications several days up to a week prior to running out of your prescription.

Controlled substances, such as medications for pain, anxiety, inattention, etc. will only be refilled by your regular healthcare provider and during regular clinic hours. Lost, stolen or otherwise unavailable controlled substances will not be replaced. Please safeguard all medications.

If you would like to make any changes to your medications, such as; increase dose, decrease dose, or stop the medication you take, try a different medication for your condition, etc., you must discuss this with your healthcare provider and an appointment may be necessary.
Financials
We are dedicated to providing our patients with the healthcare they need and as affordably as possible. Our fees are based on a number of different factors and are inline with what is considered usual and customary for our area.
We participate with the majority of health insurance companies. However, since there are hundreds of individual health plans it is your responsibility to verify our participation with your plan and understand your insurance benefits and coverage limitations.
Insured Status

NO health insurance (self-pay) - Payment in full is expected when services are rendered.

Health Insurance - We will submit claims on your behalf to both primary and secondary insurances. Co-payments or deductible amounts are due when services are rendered.

Medicare, Tricare, Medicaid - If your particular plan has a declared co-pay amount, we collect that at the time services are rendered. If your plan requires you to pay a percentage, we will send you a billing statement once your insurance claim processes.

Workers Compensation/Auto Insurance - We will bill the carrier only when we have been provided with a claim number and other necessary information to do so. Because these issues occasionally require significant follow-up, we ask that you monitor your claim. Any claim greater than 60 days old will become patient responsibility.
Payments

We accept cash, personal check, all forms of credit card and HSA cards. Payments may be made directly to our office or via our secure Payment Portal if you would like to register for access.

A $30 fee will be assessed for NSF returned checks.

While payment is expected when services are rendered, we do understand that sometimes that just isn't possible. We have several payment plans available should the need arise. Please ask to speak with our billing coordinator if you would like to make arrangements to pay your balance.
